It is distinguished from its nearest congener, Cobitis biwae — Japan's most widespread and taxonomically complex spined loach, a species-complex notorious for extensive hybridization and tetraploidy among its regional forms — by a specific combination of characters: a suborbital spine with three pointed ends, a kitchen-knife-shaped lamina circularis (the internal bony plate that anchors the defensive spine), a Z2 body marking nearly as thick as the eye diameter in many individuals, and a distinctive pattern of dark and pale spots layered at the base of the caudal fin.
The paper's own reference list is worth noting: it repeatedly cites Takahashi's earlier work on an unnamed Kochi Prefecture population referred to only as "Cobitis sp. 2" (Kochi Prefecture Red Data Book, 2002) and later "Cobitis sp. Morphology
The original description diagnoses Cobitis tosaensis against C. biwae using a tight set of characters rather than a single obvious feature: a suborbital spine (the erectile defensive spine that gives spined loaches their name) ending in three points rather than the arrangement typical of its relative; a lamina circularis — the bony plate the spine folds against — shaped like a kitchen knife; a Z2 marking (one of the rows of dark zigzag body markings long used to separate similar-looking Cobitis species) that runs almost as thick as the eye diameter in many individuals; and a caudal-fin base carrying two black spots on the surface layer, usually separated from the fin's upper and lower edges, over two lighter greyish spots on a deeper layer that are likewise separated from the edges.
