Taxonomy & naming
Otopharynx pachycheilus was described in 2001 by Matthew Arnegard and Jos Snoeks. The genus Otopharynx was erected by Charles Tate Regan in 1920 for a group of laterally spotted Lake Malawi haplochromines, and has since been revised several times, notably by Eccles and Trewavas in their 1989 classification of the lake's non-mbuna cichlids and again by Michael Oliver's 2018 description of six additional species. The genus name combines the Greek ous/ōt-, “ear,” with pharynx, “throat,” an allusion to a cranial or pharyngeal feature noted in the type species. The specific epithet pachycheilus combines the Greek pachys, “thick,” with cheilos, “lip,” describing the species' hypertrophied, fleshy lips.
Morphology
The defining feature of Otopharynx pachycheilus, as its name states outright, is a pair of markedly thickened, fleshy lips — a trait that recurs convergently in several unrelated rock-dwelling cichlid lineages across Africa's Rift lakes (for example in Lake Tanganyika's Lobochilotes and in various Lake Malawi thick-lipped mbuna), typically associated with feeding in narrow rock crevices. Beyond the lips, it shares the generally deep-bodied build and laterally spotted pattern typical of Otopharynx. Specific meristic counts and colouration details for this species have not been confirmed here.
Habitat
Otopharynx pachycheilus is associated with Lake Malawi's rocky habitats, where hypertrophied lips are thought to aid in extracting food from crevices between and beneath rocks — a niche shared with other unrelated thick-lipped Rift lake cichlids. Feeding

Otopharynx pachycheilus is presumed to be a maternal mouthbrooder, following the near-universal pattern of Lake Malawi haplochromines: the female takes fertilised eggs into her mouth and incubates them, along with the resulting fry, before releasing free-swimming young.
