Lethrinops macracanthus belongs to the genus Lethrinops, erected by Charles Tate Regan in his 1922 revision of the cichlid fishes of what was then Lake Nyasa, to house the lake's sand-dwelling, opercular-spotted haplochromines. The genus today comprises roughly twenty formally described species, plus a substantial number of undescribed forms still being resolved through combined morphological and genetic work, and macracanthus is accepted among these as a valid species in the Catalog of Fishes, the authority this atlas follows for Malawi cichlid nomenclature. The species epithet, from Greek makros ("large") and akantha ("spine" or "thorn"), most likely refers to a prominent dorsal, anal, or pelvic fin spine used as a diagnostic character separating this species from its numerous close relatives — the kind of fine anatomical detail that Lethrinops taxonomy has increasingly come to rely on as more populations across the genus are described and compared.

Where kept, husbandry should follow the standard template for a sand-dwelling Lethrinops: a deep bed of fine sand — ideally 3–4 in or more — across a substantial open floor area, since this is a committed sand-sifter that needs room to forage naturally; coarse gravel is unsuitable and can damage the gills during sifting. A footprint of 47 in or more suits a small group, with any rock structure kept minimal and pushed to the tank margins to leave the centre open.

Water chemistry should match Lake Malawi's hard, alkaline profile — temperature roughly 75–82 °F, pH 7.7–8.6, and moderate to high general hardness. Diet should be modest in protein and fat to avoid predisposing the fish to Malawi bloat, favouring small invertebrate foods such as chironomid larvae, brine shrimp, and daphnia over rich, meaty prepared foods. Lethrinops species are generally peaceful and should not be housed with aggressive mbuna, which will outcompete them for food and stress them through territorial harassment; calmer open-water or sand-dwelling tankmates are a better match.
